File A Consumer Prescription Drug Complaint

IF YOU ARE AN INDIVIDUAL CONSUMER FILING A PRESCRIPTION DRUG COMPLAINT AGAINST AN INSURANCE COMPANY OR PBM:

You should first file an appeal with the Insurance Company or Pharmacy Benefit Manager (PBM).

  • If denied, fill out the Individual Consumer - Prescription Drug Complaint Form in its entirety and provide:
    • Appeal denial documentation from the Insurance Company or PBM.
    • Your Signature on the Complaint form.
  • The Complaint will be reviewed to see if the Alabama Department of Insurance has any regulatory grounds to file the Complaint with the Insurance Company or PBM.
